I am a 35yr old male with a severly receeding chin/lower jaw. This not only looks terrible, but I'm told it probably causes my severe snoring. Can this be corrected by surgery, is it safe procedure, are there risks, do I have to get my teeth realigned first, if I get the procedure would there be scarring?

Jeff would be better off calling a local Oral Surgeon in your area to answer your questions. I cannot make an opinion on your case without the use of xrays, and a complete exam. The best I will say is yes,jaw surgery can correct a deficient jaw, and yes,there are risk which any patient must be aware of prior to starting treatment, and yes, often it is done in conjunction with orthodontic treatment, and no, often there are no external scars.

Hi,
I was wondering with high FSH levels of 15 and 17 if I could still become pregnant on my own? I've gone to an RE for months without any success doing clomid, injectables, iui's. I'm doing acupunture and herbs at the moment and feel so much better with this treatment than with the drugs. My insurance dosen't cover any infertility so I'm not able to go to the RE anymore. Is it at all possible I could become pregnant without invasive treatments? I have a two year old son that I had no problems conceiving. Thanks for your response!
Answer:
It's possible to conceive with your FSH levels, although the likelihood is low. Also, you would be at a very increased risk for miscarriage, due to diminished egg quality.

Hi. I'm a 30 year old African-American woman who has been battling acne and espcially hyperpigmentation associated with acne scarring since I was a teenager. I've been to dermatologists and aestheticians and received accutane, antibiotics and a couple of years worth of chemical peels and micro-dermabrasion with little results. I'm interested in getting rid of this problem once and for all! My question: Is a laser procedure a safe and viable option for Black skin with my condition and how much does such a treatment run? My complexion is medium-brown and I was told a couple of years ago that lasers were not recommended for my skin tone, so I'm wondering if the technology has improved since then. If not, what other options do I have?

Dear Rae,
Lasers procedures work very well on your skin color and I perform many. In my opinion, it is the best means of evening out your pigmentary problems as well as improving the scarring. The trouble is that not very many physicians have experience with non-white skin in laser resurfacing. I have presented my techniques at many national and international meetings and am currently publishing a paper on the matter. There is no question in my mind that you can be greatly improved with a laser resurfacing procedure.

I am a 32 year old female with a 3 year old son. I have been trying to get pregnant again since Dec 1999 with a misscarrage in my 7th week in January 2000, and another in my 5th week in May 2000. (both of these had neg. home pregnancy tests, only a blood serum test showed positive) and have been unable to conceive since then even using ovulation tests (showing positive results) I am also experiencing horrible symptoms which I believe may be related to low progesterone levels in the last 2 weeks of my cycle such as depression, rage, sugar cravings, bloating, breast tenderness, and fat gain. I saw my doctor yesterday because the symptoms (mainly depression and rage) are becomming overwhelming. She recommended only that I use an anti-depressant. I don't believe that this solves the problem, and it doesn't address my "infertility" and other symptoms. My doctor also stated that a saliva test will not give a true reading of my hormone levels (she didn't seem to know all that much about progesterone problems-if there is such a thing) How can I find out if my problem is caused by estrogen-dominance, or if it's a chemical imbalance? Do I really need to have another misscarrage before I can see a specialist? PLEASE HELP ME, I'm so frusterated by all the roadblocks.

You desperately need to see a Board Certified Reproductive Endocrinologist for an expert opinion on the reason(s) for your losses, rapid diagnosis, and timely treatment! You should NOT have to suffer another loss, and since there are MANY potential causes of recurrent pregnancy loss, it's time for a comprehensive evaluation and therapy plan.
If you need the name of an RE in your area, please write back with the name of the largest and closest city/state.
Just to let you know, I recommend blood testing over saliva testing for hormone analysis.

On your list of doctors I wasn't sure who to ask so...I have had a planters wart for three years and have tried everything to get rid of it. Acid, lazer, anti cancer drug, scrapping, but nothing has helped. The doctor suggested surgury, but said I would have lots of scar tissue. Is there anything I can do or anyone I can write to to ask what else I may try. I am in pain and it encompasses most of the ball of my foot. I don't know if you can help with this question, but if you know who I could ask could you please let me know. Thank you

I have found that the best treatment for severe plantars warts is a combined treatment. What I have my patients do is to use commerically available wart remover but use it in the following manner. Apply one drop on the area then apply very strong tape (like mole-skin) over the drop. The next day, tear off the tape and put another drop of the solution on. Keep doing this until the entire area turns white. It will then be possible to remove the thickened skin. Then continue with the same process until all of the thickened skin is gone and you will see the tiny capillary bed where the wart virus lives. There should be many tiny dark spots at that level. At that point I have my patients come to the office and I use a KTP laser to ablate the individual nests of virus. I have found this treatment very useful so as not to cause scarring on the bottom of the foot.

The doctor-patient relationship requires that both parties are comfortable with each other. If your doctor is not comfortable with your marital arrangement, I would find a doctor that is. After 2 miscarriages, it is most likely that everything will go fine without treatment.

It is hard for me to believe that there are ophthalmologists who do not know that the bitoric treatments give a far superior visual result than "double carding." There are some cases when double carding may be appropriate, but this is the exception. In our practice the patient's best interest is our chief motivation for recomending procedure(s). We place patients with your refractive error on a list so we can treat them the way we would want to be treated. We are not desperate for surgery to do, and we are not motivated by our own economic considerations. I am sorry you are having an outcome that is less than what you could have had. I would go to another surgeon for the rest of your care.

It is an x-ray and dye study of the uterus and fallopian tubes and is needed evaluate your uterine cavity prior to treatment.
I am concerned that you are proceding with IVF with a husband with a chromosomal disorder. THis can doom any pregnancy that occurs to miscarriage and you can also transmit this translocation to your offspring. Make sure that you have spoken to a genetics specialist prior to beginning treatment.

Hey DocI seem to have allergic conjuctivitis per my doctor. Could house dust mites or cockroach droppings cause this type of reaction? Is there something besides cleaning that I can do to help relieve this problem? This problem started back in February suddenly. Two things happened, one I bought color contact lenses over the counter. I thought I lost one,but 2 days later it came out of my eye. I had do idea it was it was there, then a couple of days later I came in contact with a parrot that my friend has as a pet. I recently heard they have a type of dandruff that may have started/caused this problem, so I washed everything in my apartment twice! Is it possible for this dandruff to remain on clothing? The answers to your questions about cause are yes, yes, and yes, but house dust mite is most likely in your case as of now.
Treatments are allergy shots with house dust mite and to discard, clean, and cover items contaminated with mite antigen, e.g. mite saliva, urine and droppings. I recommend seeing an allergist to establish a firm immunologic diagnosis.
Question:
I used a biore pore cleaning strip and the strip removed a piece of skin from my forehead. After two and a half weeks, the wound was essentially healed, but the area was still intensely red. I went to a dermatologist who used a pulse laser to treat it. She said the area would become grayish purple and then, in 7 - 10 days, it would fade. The redness, too, would fade. She also recommneded that I use Mederma lotion on the area. It is now two days later and the area is redder than ever--with no sign of purple or gray. I called the office and was informed that the treatment would still be effective--and that I could expect the same results as predicted before--that is, marked improvement in about two weeks. I am worried,however, about the increased redness. Can you explain this? Can you reassure me that everything is going as planned? Many thanks for your help.

Unfortunately, I would have liked to see the wound prior to the use of the laser. Post-op wound care as per the dermatologist is recommended. Redness does not mean necessarily that there will be scarring. Just the healing process mostly. Good luck. If you are in our area, give us a call.
