I am a 20 year old girl with a big problem--make that 2 big problms. I am 5'1", 110 pounds--and my breast size is 32 DD. I've been a DD since I was 16 years old. Having large breasts effects every aspect of my life, I dread going up and down stairs, running, etc. I am a college sophomore who is claimed as a dependent by my parents. My mother is highly opposed to a breast reduction--and I've wanted one since I was 14 years old. I have constant back pain, neck pain, shoulder pain, etc. I know that I might not be able to breast feed and I know that there will be scars. But I also know that getting a breast reduction is something that I WILL do eventually. I just need some advice. Should I go ahead and do this now or should I wait? And if I do decide to wait, are there any options to help with the symptoms of having large breasts?? Thank you!

Answer:

Only you can decide whether or not to have breast reduction, and you are old enough to make this decision yourself. You certainly have sufficient problems to have it done now and I can see no advantage to delay. There is no non-surgical way to make them smaller. It is possible that liposuction of your breasts will make them lighter, though liposuction of the breasts is often less effective in someone your age than in an older woman. David M. Metzner, MD

would like to tighten my neck, its a little saggy...what does it entail...thanks

Answer:

Dear Gary, If the excess is mainly skin, what is entailed is a neck lift with essentially the same incisions and down time (about 2 weeks) as a facelift. Often the muscles of the neck also need to be addressed and this is done through a small incision under the chin. Liposuction and "week end" facelifts will not help with the skin excess.

Question:

I was wondering, how to get rid of my double chin? How long do they put you out for surgery? What are the risks? And how much does it cost? How long does it last. Can you exercise your chin and get the same results?

Answer 1:

It depends on what is composing the double chin----fat, skin, muscle. Usually the best results are achieved with operations that attack what ever is causing the excess. Sometimes just liposuction will work. Sometimes liposuction with muscle tightening. Sometimes a neck lift, sometimes a face lift. It is a good idea to seek out a qualified plastic surgeon and let him/her examine you. No exercise will reduce the redundancy.

Answer 2:

Dear Susan, The treatment depends upon what the problem is and the condition of your skin. If there is a lot of skin excess, a neck lift is necessary which is an operation which takes at least 3 hours and for which the recouperation is about two weeks. If liposuction is required, the operation may just take an hour and the recovery period is much less. Sometimes, an abbreviated combination of both procedures can be the best choice. Exercise will not help.

Question:

I had facial plastic surgery in 1996 with disasterous results. My doctor without my consent or knowledge liposucked my entire face and neck out. I looked dreadful; anorexic and skeleton like. Why is this mutilating procedure being taught in some of our best med schools? My doc was a med student at Loma Linda Medical School. I have had some lipostructure done in N.Y. with some success. What was done to me was criminal! J.W.

Answer:

Dear Joanne, I could not agree more. The fatty layer of the face is a sign of youth and not aging and most of us are going to very conservative fat removal if any at all. Liposuction of the face is very rarely performed by cosmetic surgeons who perform a great deal of facial plastic surgery. The place where your doctor was a medical student is much less important than where and what type of residency and fellowship training was performed. Many ENT and dermatologists perform plastic surgery and many of them do so well. Even plastic surgeons, should have extra training in cosmetic surgery prior to performing procedures - most plastic surgical residency training is reconstructive. I hope that your fat grafts take well although that is a very "iffy" procedure. There should be some soft-tissue fillers here soon that will offer some long-term help to your problem.
