AS this lady had undergone a gallbladder procedure using a large incision (before the days of laparoscopic surgery), it was risky to do the standard tummy tuck. Lifting up the skin from the underlying muscle, divides some blood vessels to the skin. The old surgical scar may have divided the remaining blood vessels that would supply the skin below the scar and therefore a piece of skin could lose its blood supply and heal in a delayed fashion with a large scar. Therefore the operative plan was modified. Extensive liposuction was done. The skin was undermined only up to the umbilicus (not above it). The muscle was repaired from the umbilicus down and the excess skin below the umbilicus was removed. Note that the old scars in the upper abdomen are in the same location as before surgery. However the old vetical scar in the lower abdomen has been removed with the excess skin.
