Our practice provides service to patients from the entire northern Midwest including Michigan, Ohio, and Ontario Canada. Our Canadian patients come mainly from Ontario including Windsor, Sarnia, Wallace Burg, Chatham, Hamilton and London. Dr. Marshall is chief of the Division of Bariatric Surgery at Henry Ford Bi-County Hospital where we have a Comphrensive Surgical Weight-Loss Program. Our brand-new 35,000 sqft. Operating Room is well-equipped to handle even the largest of patients. Our Anesthesiologists are very experienced in the care of bariatric patients. We also have a brand new 12-bed state of the art ICU. Our dedicated Bariatric Unit is the only one of its kind in Macomb County and provides a pleasant surrounding for our patients with large private rooms, special bariatric hospital beds, large chairs and specialized nursing care dedicated to the needs of our bariatric patients. Henry Ford Hospital has a full array of services to provide the care needed for our bariatric patients. Specialty areas are covered with available Internal Medicine, Cardiology, Pulmonary, Infectious Disease, Intensive Care, Rehabilitation Medicine, Dietary, Pharmacy, Radiology and Emergency Medicine etc. Morbid Obesity is a very common problem in the USA as well as the rest of the developed world. Patients interested in Bariatric Surgery need to be well-informed to make sure that this is the right choice for them. We have monthly Education Seminars as well as Pre-Surgical Education Classes and Support Group Sessions. We provide much of the information to our patient during our office surgical consultation. We work along side the primary care physicians who assist us with thorough pre-operative evaluations. Dr. Marshall sees all of his patients in the office for follow-up care where they get the personal attention of the Surgeon. We plan on life-long follow-up with at least yearly visits to check necessary laboratory testing and to make sure our patients stay on track with necessary vitamins, minerals and life-style changes. It is crucial that patients see a Bariatric Surgeon knowledgeable in the follow-up care of bariatric patients to make sure they have continued successes. Dr. Marshall performs both Open and Laparoscopic Roux-En-Y; Gastrojejunostomy (Gastric Bypass). This includes both short and long limb procedures needed for larger patients with a Body Mass Index (BMI) over 50. Dr. Marshall also is experienced in performing bariatric Revisional procedures which is usually a repair or conversion of a failed previous bariatric surgery. Many of the earlier operations such as Gastroplasty (stomach stapling) and Lap-Band are unsuccessful and we usually convert to a Gastric Bypass. There are many other operations which have been used for surgical weight loss such as the Intestinal Bypass (now no longer performed due to side-effects), Biliopancreatic Diversion (BPD) and Duodenal Switch. These are mostly malabsorptive procedures that have long-term complications regarding nutritional deficiencies. Gastric Bypass is a much safer procedure with exceptional results and a low incidence of short and long-term complications. After the weight loss, many patients are interested in having the extra skin and fat removed from the abdomen, upper arms, legs and other areas. Dr. Marshall has had considerable experience in performing Abdominoplasty (removal of excess skin from the lower abdomen), Brachioplasty (removal of excess skin from the upper under arms) and Thigh Plasty (removal of excess skin from the upper, inner thighs). We are associated with other plastic surgeons who perform all of the other plastic procedures patients may seek after extreme weight lost such as breast-lift (mastopexy), augmentation (breast implants), breast reduction, facial plastics, liposuction and other procedures. Dr. Marshall performs his own endoscopic procedures such as esophago-gastroendoscopy (EGD, EGJ), and colonoscopy. He is one of the only surgeons in the area to perform Endoscopic Sclerotherapy for dilated stomas (enlargement of the stomach-pouch opening), which can be a cause of weight regain and insufficient weight-loss after some types of gastric bypass procedures. Dr. Marshall also treats some gastric fistulas with an endoscopic sealant which is helpful in some cases. Dr. Marshall also performs many other bariatric-related procedures. Inguinal Hernia Repair Gastric Bypass is the gold standard for bariatric surgery and combines both a restrictive component with a malabsorptive component which results in optimum weight loss with few complications. Expected weight loss with a gastric by-pass is 60-70 percent of the excessive weight will be lost. Our patients commonly loss 75-85 percent of their excess weight in the first year and many patients continue to improve this up to 2 years. Long term results of gastric by-pass have been consistent for the past 20 years. Any weight regain is usually minimal (10-15 pounds) - any more than this may suggest that some problem exists such as stomal dilation (where the pouch opening is too large), pouch dilation (when the pouch stretches too large) or gastric fistulas (when there is an abnormal opening into the old stomach). All of these are treatable.
